Dieser Fallbericht ist ein Leitfaden, wie Mini-Implantate als strategische Pfeiler für die Stabilisierung von herausnehmbaren Teilprothesen und als Stütze für die verbleibenden Zähne verwendet werden können.
Einführung: Die Verwendung von konventionellen Zahnimplantaten als strategische Pfeiler für bestehende herausnehmbare Teilprothesen ist kostenintensiv und erfordert eine ausreichende Breite des Kieferkamms. Die einteiligen Mini-Zahnimplantate (< 2,8 mm) bieten eine alternative Behandlungsmethode für diese Indikation. Behandlungsmethode: Ein Therapieschema für strategische Mini-Zahnimplantate (MDI 3M ESPE Dental GmbH, Seefeld, Deutschland) beinhaltet 3 Pfeiler (Zähne + Implantate) pro maxillarem Quadranten und 2 Pfeiler pro mandibulärem Quadranten.
Ergebnisse: Im ersten Fallbericht hat eine Patientin eines ihrer 2 Eckzahn-Konuskronen-Abutments verloren. Anschließend wurden 2 Mini-Zahnimplantate mit ausreichender Primärstabilität eingefügt (Insertionsdrehmoment > 35 Ncm) und sofort mit den Matrizen unter Verwendung von intraoraler Polymerisation belastet.
Die zweite Patientin hatte 2 verbleibende Backenzähne. Nach Platzierung von 4 MDIs (Insertionsdrehmoment 20-25 Ncm) wurde der herausnehmbare Zahnersatz, der durch Konuskronen gehalten wird, mit weichem Material unterfüttert. Vier Monate später wurden die Matrizen im Dentallabor über Druck in den Zahnersatz polymerisiert.
Schlussfolgerung: Zusätzliche strategische Pfeiler unter Verwendung von Mini-Zahnersatz erfordern ein Befolgen der Empfehlungen bezüglich der Anzahl und Platzierung von MDIs, ein Belastungskonzept, anatomische Kenntnisse, chirurgische Erfahrung, vor allem bei der Bearbeitung des Kieferknochens, und ausreichende prothetische Erfahrung.
