Feste Vollbogen-Rekonstruktionen durch geführte Implantatchirurgie und Sofortbelastung der Implantate: eine retrospektive klinische Studie an 12 Patienten mit 1-jähriger Nachbetreuung

Abstract – Hintergrund
Geführte Implantatchirurgie gilt als ein sicheres, minimalinvasives Verfahren ohne Lappenbildung. Dennoch wurden die geführte Chirurgie ohne Lappenbildung, die Positionierung von Implantaten in Extraktionsalveolen sowie die Sofortbelastung von festen Vollbogen-Rekonstruktionen ohne Zahnfleischmaske noch nicht umfassend ausgewertet. Absicht dieser retrospektiven klinischen Studie war es, die Überlebens- und Erfolgsrate von festen Vollbogen-Rekonstruktionen ohne Zahnfleischmaske zu dokumentieren, die per geführter Chirurgie und Sofortbelastung von Implantaten, selbst in frischen Extraktionsalveolen, erzielt wurde.

 

Schlussfolgerungen
In unserer vorliegenden Studie erhielten 12 Patienten 110 Implantate (65 davon post-extraktiv), die ohne Lappenbildung mittels geführter Chirurgie platziert und daraufhin mit einem provisorisch befestigten Vollbogen sofort belastet wurden. Nach einem sechsmonatigen Provisoriumszeitraum wurden 72 feste prothetische Zirconiumdioxid-Restaurationen (53 Einzelkronen, 17 Brücken sowie 2 feste Vollbögen) bereitgestellt. Die Ergebnisse zeigten eine einjährige Implantat-Überlebensrate von 98,2 % (108/110 überlebende Implantate) und gute Weichgewebestabilität.

Unsere derzeitige chirurgische und prothetische Herangehensweise weist diverse Vorteile auf. Zunächst wird nur eine chirurgische Sitzung für die Zahnextraktion, Implantation und die Befestigung des Provisoriums benötigt. Für das gesellschaftliche Leben der Patienten bedeutet dieses Konzept eine Verringerung der Beschwerden und eine weniger komplizierte Rückkehr ins Berufsleben. Hinsichtlich der dentalen Rehabilitation unterstützt die provisorische Restauration die Heilung des Weichgewebes für ein optimales ästhetisches Ergebnis. Innerhalb der Grenzen dieser Studie scheint eine Kombination von chirurgischer Führung mittels CBCT zwecks sofortiger Platzierung des Implantats in Extraktionsalveolen in Verbindung mit dem sofortigen Einsetzen des Provisoriums und dessen Belastung in den angezeigten Fällen eine sichere und vorhersehbare Behandlung mit hohen Überlebensraten und ausgezeichneten ästhetischen Ergebnissen zu sein. Es werden weitere Studien an größeren Patientengruppen und mit längeren Nachkontrollen benötigt, um spezifischere Schlussfolgerungen über Langzeitergebnisse mit dieser Technik zu ziehen.
